Pain or stiffness through the middle of the back can make twisting, reaching, sitting, working, and even taking a deep breath uncomfortable. The middle—or thoracic—region of your spine extends from the base of your neck to the lower back and connects with your rib cage. Because so many joints, muscles, and ribs work together in this area, there can be several possible sources of discomfort.

What Can Cause Mid Back Pain?

Mid-back pain may begin after an injury or unusual activity, but it can also develop gradually.

Common factors may include:

  • Muscle or joint irritation
  • Restricted or uncomfortable movement
  • Repetitive activities or physical work
  • Prolonged sitting or sustained positions
  • Lifting, twisting, or reaching
  • Sports or exercise-related injuries
  • Rib or chest-wall musculoskeletal problems
  • Injuries from falls or auto accidents

Sometimes discomfort in this area is influenced by movement in the neck, shoulders, ribs, or other parts of the spine, so we consider more than just the spot that hurts.

What Does Mid Back Pain Feel Like?

Symptoms can vary depending on what's contributing to the problem. You may notice:

  • Aching or soreness between or around the shoulder blades
  • Stiffness or difficulty twisting
  • Pain with certain movements or positions
  • Muscle tightness or spasm
  • Discomfort around the ribs
  • Pain associated with reaching or taking a deep breath

Not every pain in the middle-back or rib area comes from the muscles or joints. Chest pain, difficulty breathing, or other unusual or concerning symptoms should be evaluated promptly by an appropriate medical provider.
