Our Cancellation policy
Your appointment time is reserved just for you. We require 24 hours notice for any cancellations or changes to your appointment so we may fill available appointment times. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a cancellation fee to the card on file.
Payment at Time of Service
A Good Faith Estimate (GFE) is an estimate from a health care provider (Blaine Chiropractic and Massage) for the expected costs of items or services. Blaine Chiropractic and Massage will make a GFE based on insurance plan information provided by you, the patient. Payment due for copay, deducible, or private pay will be collected on the same day of service. Your estimate is based on services expected to be provided and is based on allowable fee amounts through individual insurance plans, and in reference to deductible amounts that may be owed through your individual plan. As an estimate it is possible that more or less than the amount collected will be owed after insurance billing. Blaine Chiropractic and Massage will invoice patients when payment is due. Overpayments can be applied to next visits or refunded by check. We regularly review accounts for overages.

Guide to Insurance Billed Massage at Blaine Chiropractic

If your carrier is listed here, please confirm that your plan includes massage benefits and answer the following questions:
– Do I need Referral or Preauthorization?
***Regence requires a prescription for massage therapy on file. A complete prescription includes a date of condition onset, diagnosis codes for treatment, and expected number of visits required for improvement of the condition.
– What are my Benefit Limits (number of treatments allowed in a cycle)?
Please be aware of your maximum authorized visits or plan limits.
For massage claims legitimately rejected by your service provider for failure to adhere to plan policies and benefit maximums, you will receive a notice for payment due at the rate billed to your insurance carrier. Please also note that missed appointment fees from cancellation within 24 hours of appointment time are not insurance billable.
VA/TRIWest: We are proud to serve our veterans with Massage Therapy care. Veterans can seek referral through a VA approved primary care provider and must contact the VA to arrange prior authorization. Our office will receive confirmation and communication from the VA to begin scheduling. Once the prior authorization has been obtained by our office and your initial visit scheduled with VA assistance we will be happy to then help you to schedule further appointments within your authorization dates.
For Personal Injury and Auto Accident: Referral Steps & Paperwork
When scheduling for Personal Injury and Auto accident cases, we require the following information: An active referral from your managing care provider recommending Massage Therapy that includes diagnosis code, the name and contact for claim manager, as well as your case number and date of injury. All required forms to begin your personal injury care will be sent via email and can be electronically submitted. Referrals can be faxed to 360-332-6071 or emailed to admin@blainechiropractic.com.
WHAT TO EXPECT
Your first Chiropractic visit is a 45-60 minute Intake, Exam, and Adjustment. Dr. Christopher will take time to listen to what brings you into our office, your conditions, health history, and health goals.
After a physical exam determining Range of Motion in the joints, strength testing, and visual analysis of posture and gait X-rays will be performed if indicated.
Our office does not use X-ray for every patient. The Intake and Exam process will also screen for any contraindication to manual adjustment, and to assess whether you would benefit from other modalities offered such as Cold-Laser, basic nutritional supplementation, or Massage Therapy.
Dr. Christopher would like to see you in the office one week after your first visit and adjustment and in this appointment your feedback and a physical assessment will allow Dr. Christopher to recommend a comprehensive treatment plan which may include additional adjustments

We offer online fillable intake forms! Once you have called to schedule your initial exam or Massage appointment simply check your email and follow the link to our forms. No printing needed as forms, consents, and signatures are submitted back to us right away! Please fill in any available insurance information and bring your insurance card and ID to your first appointment. The forms listed below may be accessed for reference or in the case that an online form is not accessible.
